PARTICIPANT WAIVER AND RELEASE OF LIABILITY
Eriselam Sport – Youth Soccer Training Program
AGREEMENT AND CONSENT
This Waiver and Release of Liability ("Agreement") is entered into by the undersigned parent or legal guardian ("Parent/Guardian") on behalf of the minor participant ("Athlete") in connection with participation in any and all Eriselam Sport training sessions, programs, events, and activities (collectively, "Program") held in Colorado.
By enrolling your child in the Eriselam Sport program, you acknowledge that you have read, understood, and agree to the terms set forth in this Agreement.
1. DESCRIPTION OF ACTIVITY
Eriselam Sport offers structured youth soccer training for athletes ages 5–17. Activities include but are not limited to: drills, scrimmages, conditioning exercises, speed and agility training, team-based activities, and any other activities associated with the Program.
2. ASSUMPTION OF RISK
Parent/Guardian acknowledges that participation in soccer and athletic training involves inherent risks, including but not limited to:
Physical injury (minor to severe), including sprains, fractures, and concussions
Illness or physical exhaustion
Accidental contact with other participants or equipment
Environmental conditions (heat, cold, uneven surfaces)
Any other unforeseen risks associated with physical activity
Parent/Guardian voluntarily accepts and assumes all such risks on behalf of the Athlete, whether known or unknown, in connection with participation in the Program.
3. RELEASE OF LIABILITY
To the fullest extent permitted by law, Parent/Guardian, on behalf of themselves and the Athlete, hereby releases, waives, discharges, and holds harmless Eriselam Sport, its coaches, staff, volunteers, directors, agents, and representatives (collectively, "Released Parties") from any and all claims, demands, causes of action, damages, losses, or expenses — including legal fees — arising out of or in connection with the Athlete's participation in the Program, whether caused by the negligence of the Released Parties or otherwise.
4. MEDICAL AUTHORIZATION
In the event of an emergency in which Parent/Guardian cannot be reached, Parent/Guardian hereby authorizes Eriselam Sport staff to seek and consent to necessary emergency medical treatment for the Athlete. Parent/Guardian agrees to be responsible for all medical expenses incurred.
Parent/Guardian confirms that the Athlete is physically fit to participate and has no medical conditions that would prevent safe participation, unless previously disclosed in writing to Eriselam Sport staff.
5. PHOTO AND MEDIA CONSENT
Parent/Guardian grants Eriselam Sport permission to photograph or record the Athlete during Program activities and to use such images or footage for promotional, educational, and social media purposes, without compensation. If you do not consent to this, please notify us in writing at [email protected].
6. CODE OF CONDUCT
Parent/Guardian agrees that the Athlete will:
Treat coaches, staff, and fellow athletes with respect
Follow all instructions given by coaching staff
Arrive on time and prepared for sessions
Avoid any conduct that endangers themselves or others
Eriselam Sport reserves the right to remove any participant from the Program whose behavior is deemed unsafe or disruptive, without refund.
7. PAYMENT AND CANCELLATION POLICY
Parent/Guardian acknowledges and agrees to the payment terms set forth at enrollment. Monthly subscription fees are due as scheduled. Eriselam Sport reserves the right to update payment terms with reasonable notice.
8. GOVERNING LAW
This Agreement shall be governed by and construed in accordance with the laws of the State of Colorado.

By signing below, Parent/Guardian confirms that:
They have read and fully understand this Agreement
They are the parent or legal guardian of the Athlete named below
They voluntarily agree to all terms stated herein
